I desire to volunteer with the Western Illinois Regional Council – Community Action Agency (WIRC-CAA) and engage in the activities related to being a volunteer for a program or project.

I fully acknowledge and understand that I am freely and voluntarily donating my time, effort and services to the Western Illinois Regional Council – Community Action Agency and will receive no compensation of any kind in return, that there is not an employer-employee relationship of any type with the Western Illinois Regional Council – Community Action Agency, and that I can choose to end my volunteer status with the Western Illinois Regional Council – Community Action Agency at any time.

I hereby freely and voluntarily, without duress, execute this Waiver under the following terms:

Waiver and Release. I release and forever discharge and hold harmless the Western Illinois Regional Council – Community Action Agency and its successors and assigns from any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, which arise or may hereafter arise from my volunteer time with the Western Illinois Regional Council – Community Action Agency.

I understand and acknowledge that this Waiver discharges the Western Illinois Regional Council – Community Action Agency from any liability or claim that I may have against the Western Illinois Regional Council – Community Action Agency with respect to bodily injury, personal injury, illness, death, or property damage that may result from my participation on the Western Illinois Regional Council – Community Action Agency program or project site. I also understand that the Western Illinois Regional Council – Community Action Agency does not assume any responsibility for or obligation to provide financial assistance or other assistance, including but not limited to medical, health or disability insurance, in the event of injury, illness, death or property damage.

I understand that any information gathered by volunteering at the Western Illinois Regional Council – Community Action Agency is all confidential and should not be shared with others. This includes, but is not limited to, personal information of those using our services.

I understand that due to the nature of WIRC-CAA’s programs, we will check online databases for registered sex offender status. Registered sex offenders cannot volunteer for any WIRC-CAA programs, for the safety of customers.

Insurance. I understand that I expressly waive any such claim for compensation or liability on the part of the Western Illinois Regional Council – Community Action Agency beyond what may be offered freely by the representative of the Western Illinois Regional Council – Community Action Agency in the event of such injury or medical expense.

Medical Treatment. I hereby release and forever discharge the Western Illinois Regional Council – Community Action Agency from any claim whatsoever which arises or may hereafter arise on account of any first-aid treatment or other medical services rendered in connection with an emergency during my volunteer time with the Western Illinois Regional Council – Community Action Agency.

Assumption of the Risk. I understand that my volunteer time with the Western Illinois Regional Council – Community Action Agency may include activities that may be hazardous to me, including, but not limited to, construction activities, loading and unloading of heavy equipment and materials, and local transportation to and from the program or project sites. I hereby expressly and specifically assume the risk of injury or harm in these activities and release the Western Illinois Regional Council – Community Action Agency from all liability for injury, illness, death, or property damage resulting from the activities of my volunteer time with the Western Illinois Regional Council – Community Action Agency.

Photographic Release. I grant and convey unto the Western Illinois Regional Council – Community Action Agency all right, title, and interest in any and all photographic images and video or audio recordings made by the Western Illinois Regional Council – Community Action Agency during my volunteer time for the Western Illinois Regional Council – Community Action Agency, including, but not limited to, any royalties, proceeds, or other benefits derived from such photographs or recordings.

Other. I expressly agree that this Waiver is intended to be as broad and inclusive as permitted by the laws of the State of Illinois in the United States of America, and that this Waiver shall be governed by and interpreted in accordance with the laws of the State of Illinois. I agree that in the event that any clause or provision of this Waiver shall be held to be invalid by any court of competent jurisdiction, the invalidity of such clause or provision shall not otherwise affect the remaining provisions of this Release which shall continue to be enforceable.

While serving in my volunteer role at the Western Illinois Regional Council – Community Action Agency:

I WILL:

Treat every guest and volunteer with dignity and respect.
Be mindful that I represent the Western Illinois Regional Council – Community Action Agency while serving.
Seek out assistance from my team leader if I need help interacting with a guest or am unable to serve a guest with dignity for any reason.
Treat the information shared by guests with the utmost confidentiality.
Follow policies, procedures and guidelines as outlined in the Operations Manual for the program in which I volunteer.

I WILL NOT:

Give my opinion about a guest's personal situation or religious beliefs.
Give any money or gift cards to a guest.
Drive anywhere with a guest, visit a guest's home, drive a guest's car, or provide transportation to a guest.
Disclose information shared by a guest or learned in any way while volunteering at the Western Illinois Regional Council – Community Action Agency.
Consume or take home any of the Western Illinois Regional Council – Community Action Agency food, office or other supplies, or equipment.
