Hope Health and Hope Senior Sign Ups

Hope Health and Hope Senior are no longer using Signup.com to schedule our resident/tenant visits. Instead, friends and family should call Hope Health or Hope Senior to schedule their visit in advance. This will help ensure proper COVID screening, distancing and disinfection by our trained screeners.

Hope Health (nursing home) Visits: 920-269-4386 and select option 8

Hope Senior (assisted living) Visits: 920-269-7667 and select option 8

Please leave a message with your name, the resident’s/tenant’s name, date and time of the visit and if another person will be joining. We will only return your call if we cannot accommodate a visit at the requested time.

Also starting June 1^st, all visitors will enter and be screened at the FRONT DOOR for Hope Health or Hope Senior visits. Door 3 will no longer be staffed for entrance.

As the weather starts to become warmer, we encourage visits outdoors with our residents and tenants. Visitors are still asked to call and schedule outdoor visits to ensure proper screening, however, more than 2 friends/family members can visit if outdoors.
