Release and Waiver of Liability in favor of the CITY OF MADISON, AL, a municipal corporation, and its elected officials, officers, employees, and board members and MADISON VISIONARY PARTNERS, INC., an Alabama non-profit corporation, its officers, directors, employees, and board members (collectively the “Released Parties”).

IN CONSIDERATION OF THE VOLUNTEER BEING ABLE to serve as a participant in a Pride of

Madison program (Adopt-A-Mile and/or Adopt-A-Park) I do hereby freely, willfully, and

without duress execute this Release and Waiver of Liability under the following terms:

1) Waiver and Release: Volunteer and/or guardian hereby release and forever discharge and

hold harmless the Released Parties from all liability and claims, demands, rights of action, or

actions, of whatever kind or nature, either in law or equity, which arise, or may hereafter arise

from the Volunteer's activities with the Pride of Madison programs. Volunteer and/or guardian

understand and acknowledge that the execution of this Release discharges and will discharge

the Released Parties from any liability or claims that either may have with respect to any bodily

injuries, personal injuries, illnesses, or property damage which may result from Volunteer

activities. Volunteer and/or guardian further understand that the Released Parties assume no

responsibility for and are not obligated in any way to provide financial assistance or other

assistance, including. but not limited to medical, health, or disability insurance, in the event of

injury, illness, death, or damage. Volunteer and/or guardian, together with their heirs, estates,

executors, administrators, assignees, and family members, agree to hold harmless the

Released Parties from all legal matters, lawsuits, or litigation arising from this volunteer

relationship.

2) ASSUMPTION OF THE RISK: Volunteer and/or guardian recognize and understand the

activities within the Pride of Madison adoption program shall include, but are not limited to,

activities such as picking up trash and debris along public roads, streets, parks, and other

public places. Volunteer and/or guardian, together with their heirs, estates, executors,

administrators, assignees, and family members, hereby expressly and specifically assume the

risk of injury or harm in these situations and release and discharge the Released Parties from

and waive any and all liability for any injury, illness, death, or property damage resulting from

the activities of volunteering in the program.

3) PHOTO RELEASE: Volunteer and/or guardian give permission to have photos and/or video

recordings taken of Volunteer for publicity purposes during program activities even though no

compensation of any kind will be granted for appearing in such photos or video recordings.
